SQL that creates the idempotency table and its expires_at index. Run it
from an application migration, not at request time.
response holds the response as JSON text rather than jsonb: jsonb
rejects the \u0000 and unpaired-surrogate escapes JSON.stringify writes,
and a completed response must always be storable and replay exactly.
