Defined in: packages/pipeline/src/helpers/behavior-entries.ts:62
First identity fixes placement; the last tuple supplies options. Bare repeats preserve options.
Pass accumulators to apply that rule across multiple entry lists — global
before and after share one set so a behavior declared in both runs once,
at its first position, without the bare repeat erasing tuple options.
